At Life Fitness Therapy, it is important for us to make our patients feel as comfortable as possible throughout their journey of treatment and healing. To help introduce our services we have put together information about the initial visit so patients will be prepared with the necessary material clothing.

Getting Started

What a Patient will Need for his or her First Visit


One of the most important ways to manage pain and mobility deficits is to actively engage in rehabilitation and exercise. Physical therapy
helps patients manage those pains by exercising weak points on their body and regaining strength in area that are in need. An exercise program should be tailored to the patient's condition and pain level, and include a combination of stretching, strengthening, and aerobic routines. Knowing the right exercises is where we can help your recovery of chronic pain. Contact us for additional information.

What to Expect on Your First Visit

1. Your Physical Therapist will talk to you about what brought you in for treatment.

2. Your therapist will perform a comprehensive evaluation that includes muscle strength, joint range of motion and mobility, soft tissue mobility, posture, body mechanics, flexibility, and functional testing.

3. Depending on the findings and goals, your therapist will explain the treatment plan and start the treatment if appropriate.

4. Follow-up appointments will be scheduled before leaving the clinic.

Forms to Download for Initial Visit

If you are able to download pdf forms, please download and print the Assignment of My Benefits Form, Medical History Form, and Patient Information Form. Once you have downloaded these forms, please complete them and bring them to your first visit. Also download the Company Policies, and Pool Policies, and Statement of Privacy Notice.
